FpML-AWG Updated Affirmation Process for discussion

The main change I have made is that I have made explicit some  
implicit stuff concerning identity and message types (xsi:types). So  
all of
the tables that follow the sequence diagrams now make explicit  
reference (using XPath) to the xsi:type field.
